The role of isolated polyethylene exchange in total knee arthroplasty
نویسندگان
چکیده
The use of modular total knee arthroplasty (TKA) implants allows surgeons to perform isolated tibial polyethylene insert exchange (IPE) while retaining well-fixed and stable components. The purported advantages of IPE include preservation of bone stock, shorter operating time, less blood loss, faster rehabilitation and lower cost. However, the indications for IPE are limited.IPE for wear and osteolysis has moderate success in the medium term but should be avoided in cases of accelerated wear. In selected cases, debridement and IPE for early infection can result in low morbidity with high success rates in the short term. IPE for arthrofibrosis has poor results.IPE should be undertaken with caution and an institutional algorithm should be followed. Symptomatic flexion instability in posterior stabilized primary total knee arthroplasty.
Flexion instability in posterior-stabilized total knee arthroplasty is a relatively uncommon but distinct problem that is often underdiagnosed and may require surgical management. This retrospective study evaluated the authors' management strategy and assessed the results of revision surgery. The authors identified 19 knees that underwent revision for isolated flexion instability after primary ...
متن کاملAn impinging remnant meniscus causing early polyethylene failure in total knee arthroplasty: a case report
The management of patients with an apparently normal functional total knee arthroplasty (TKA) suffering from unexplained persistent pain and swelling is a challenging issue. The usual causes of pain after total knee replacement are well known, but there are a small number of patients in whom its aetiology is obscure. Malfunction due to soft tissue impingement has rarely been reported. A patient...

The modularity of total knee arthroplasty prostheses allows for a polyethylene insert exchange in selected knees that have isolated polyethylene wear but well-fixed and wellaligned components. When this procedure is performed, it is typically straightforward and leads to low patient morbidity. However, there are conflicting reports concerning the efficacy of an insert exchange (Acta Orthop. 200...
متن کاملPolyethylene Tibial Post Fracture in a Posterior Stabilized (Genesis II) Total Knee Arthroplasty
Posterior stabilizing knee prostheses have been increasing in popularity recently due to some studies showing excellent long-term survivorship, more consistent kinematics, technical ease, and higher range of motion.1,2 The posterior stabilized knee relies on a cam and post mechanism to replicate native femoral rollback. One of the major potential complications of this system is post fracture. T...
متن کاملAcute, recurrent total knee dislocation: Polyethylene dislocation and malreduction
A 62-year-old man underwent total knee arthroplasty using a mobile-bearing prosthesis. Four days post-operatively the patient experienced the first of several acute knee dislocations. Closed reduction was performed at an outside hospital a total of three times prior to presentation at this institution. A two-stage exchange of the TKA was recommended due to the clinical suspicion for an infected...
